How Are Services Funded?
Neuro-Rehab Management’s services are funded by private insurance, worker’s compensation, state agencies, private pay, and through affiliations with various advocacy and medical provider agencies. NRM is a certified state vendor.
Medicaid does not reimburse for NRM services. Medicare does not fund for NRM services.
Who Are The NRM Staff?
Neuro-Rehab Management case management and life skills staff consists of professionals with backgrounds in rehabilitation counseling, social work, psychology, education, and nursing. Staff comes from various backgrounds including vocational rehabilitation, medical case management, post acute case management, homeless services, and from supportive living programs and inpatient settings.
Neuro-Rehab Management also provides consulting neuropsychological services.
What is the Eligibility Criteria for Services?
All individuals referred to NRM for services must have some type of neurological illness or injury or other type of learning difficulty. NRM provides services to individuals generally over the age of 18, but we have provided services to high school age students. Other eligibility criteria includes: medically stable, not a danger to self or others, and a willingness to work in partnership with NRM staff.
How Do I Make a Referral to NRM?
Telephone the NRM office (781-979-0018) and ask to speak with someone to make a referral.
Information will be gathered via the telephone call.
Information about NRM will be sent to the individual making the inquiry. As appropriate, a meeting to further discuss the individual’s appropriateness for NRM services will be scheduled.
Prior to beginning services, a Proposed Service Agreement may be developed outlining services to be provided, projected time frames, and costs. This Proposed Service Agreement must be received prior to beginning specific services with NRM. Medical records are also gathered during this interim period of time. Please note, not all services require a Proposed Service Agreement prior to initiating services.
What makes Neuro-Rehab Management’s Services Unique?
NRM’s case management services are community-based. Our services are individualized and are provided to individuals in their homes, on their jobs, or in the community. We specialize in working with individuals with neurological illness or injury and/or other neuromuscular disability. NRM provides case management services to individuals who are homeless with brain injuries.
NRM staff also provides functional hands-on assistance in the home to assist with home organization and study habits.
As a NRM Client, What Can I Expect?
An assessment of your needs
Development of an individualized treatment plan and/or case management plan with specific goals identified
A community-based, functional approach to case management
Case managers with experience in the field of brain injury and knowledge of resources in the field
Service plan meetings and/or family meetings, as appropriate.
NRM staff who believe in the interdisciplinary team approach
Is There a Waiting List for Services?
