Case Management:

Neuro-Rehab Management’s philosophy of case management is interdisciplinary, individualized, and holistic. Case management services are designed to meet the unique needs of individuals who have sustained a neurological illness or injury, learning impairment, or other type of neuromuscular disability. We identify and build upon an individual’s strengths in establishing short and long-term goals. The functional areas which are assessed prior to beginning case management services include: medical status, physical and cognitive functioning, independent living skills, emotional /behavioral and psychosocial skills functioning, recreational, and vocational areas of life. Following the case management assessment, specific goals, recommendations, and resources are identified.

Case management services can be short or long-term. They may involve ongoing coordination and communication with treatment providers. Through the provision of case management services, there is ongoing advocacy and follow-up.

Case management services are provided to individuals referred from insurance carriers, attorneys, state agencies, professionals in the field, and by family members and individuals who have sustained the injury or illness.
Homeless Case Management:

Neuro-Rehab Management provides case management services to individuals who are homeless with brain injuries in the Greater Boston area. A primary focus of homeless case management services is to assist individuals in transitioning out of homelessness to stable housing. NRM’s case managers assist individuals in accessing medical, rehabilitative, psychiatric, and substance abuse services in order to maximize successful placement in stabilized housing.

Homeless case management services are provided to individuals and/or families referred from homeless service providers, outreach workers, social service providers, state agencies, family members, and individuals who are homeless.

Assessment:

Neuro-Rehab Management’s case managers provide assessments of an individual’s independent living skills as well as, a more comprehensive assessment of need areas. The assessment may be used to assist in discharge planning or in planning for a transition from a residential or institutional setting to the community. Prior to any ongoing case management, NRM staff provides an initial assessment of an individual’s problem areas resulting from the injury or illness and makes specific recommendations for services and interventions. Resources are identified and developed as part of the assessment process.

File Reviews:

Neuro-Rehab Management provides a review of client files to assist an attorney and/or insurance carrier in developing a proposed plan of care and resource identification and development.

Life Care Planning:

Neuro-Rehab Management’s case managers develop life care plans for individuals with catastrophic injury to assist a referral source in planning for an individual’s life needs.

Organizational Skills Training:

Neuro-Rehab Management provides in-home organizational skills training to individuals with a neurological illness or injury and/or other learning/cognitive impairments. These services may include assistance in developing schedules, identifying and utilizing compensatory organizational strategies, and in problem solving issues relating to completing paperwork. Organizational skills training may also involve assisting individuals in utilizing study skills when attending school or in preparation for examinations. 

Supportive Living Services:

Neuro-Rehab Management provides case management and community support services to individuals living in their own apartments/home. These services may include hands-on functional assistance in the home and community, assistance in accessing the community, and in developing compensatory strategies, budgets, and community resources. These services may also involve assistance in completing paper work and in applying for entitlement programs.

Vocational Consultation:

Neuro-Rehab Management provides vocational assessments, job development, job coaching, and placement services. Vocational services are provided to individuals referred from state agencies, insurance carriers, attorneys, professionals in the field, and by individuals and family members.


Head Injury Community Services:

Head Injury Community Services (HICS) is funded by the Brain Injury and Specialized Community Services Department (BISSCS) of the Massachusetts Rehabilitation Commission. The HICS program is located in Quincy, MA. HICS staff provides opportunities for consumers to develop and build on a variety of life skills; important to each individual’s needs as indicated in the Individualized Service Plan (ISP). Consumers have the opportunity to develop these skills in their home, community, group setting and on a one-to-one basis whenever possible. These skills include: independent living skills, community living skills, social skills, recreation and leisure, prevocational skills, health and wellness, and case management services.

To be eligible for this program an individual must be at least 22 years of age, medically stable, continent of bowel and bladder or have a personal care attendant available to meet these needs, and to be able to functionally benefit from the services provided.

Referrals to this program are made by The Brain Injury and Specialized Community Services Program of the Massachusetts Rehabilitation Commission. This agency also determines eligibility.

Recreation Program:

The Head Injury Community Services Recreation Program is designed to provide social and recreational opportunities for individuals with a traumatic or acquired brain injury. The program offers structured leisure activities on-site in Quincy and in the community. Events are held one evening per week and one weekend day per month. It does not cost anything to join this program. However, participants are expected to pay their own way to events, unless otherwise specified.

All participants must be at least 18 years old, have a brain injury, be medically and behaviorally stable, independent in self care or have a personal care attendant available to assist in meeting these needs at events, have transportation to and from the program site and/or activity, and be able to independently manage money in the community. Some activities that are offered include (but are not limited to) bowling, billiards, mini-golf, dinner out, game night, pot luck, movies and concerts.
